Who We Are at Crumble Cares Children and Family Services LLC

Who We Are

Crumble Cares Children and Family Services LLC, is approved, through DMHA, to provide Child Mental Health Wraparound Program and Child Mental Health Initiative services for youth Habilitation (HAB), Family Support and Training (FST) and Respite care services. Crumble Cares Children and Family Services, LLC. works together with youth and families to create and implement attainable plans that boost family success.

We pride ourselves on creating a safe, fun environment for children to learn, thrive and grow. We will be supportive advocates for individuals and families that result in the betterment of their lives.

About the Provider

For the last 15+ years, I have served as a Social Services Coordinator, a Home-based case manager, a Case Manager working with families receiving Family Support Waiver services and families referred by the Department of Child Services, Marion County Probation and Choice Coordinated Care solutions. I specialized in working with families of children with mental health diagnosis, developmental delays and medically fragile children with an average weekly full caseload. I was responsible for collaborating with interdisciplinary teams to coordinate, evaluate and monitor treatment plans to ensure successful completion of treatment plan goals for every family. As a longtime Case Manager working with families in crisis, I understand the importance of providing compassionate and personalized care. I've received extensive training in conflict resolution and de-escalation techniques.  I pride myself on my ability to develop trusted relationships with families. 

Services

Habilitation Services

Habilitation services are one on one skill building services for at risk youth. Habilitation service providers work on teaching emotional management skills, communication skills, building self-esteem and empowering youth to reach their fullest potential in a positive and productive manner.


Family Support and Training services

Family Support and Training Services are for family members and extended family supports to address how to improve overall family functioning and how to support the youth and family in a way that allows them to be successful in the community, home and school.


Respite Services

Respite care provides parents and caregivers with childcare services that offer short-term (daily or hourly) relief.


Child Mental Health Initiative Information

The Indiana Department of Children's Services’ Children’s Mental Health Initiative is designed to help families who are caring for a child, between 6-17 years old, who is diagnosed with at least 2 mental illnesses. The Children’s Mental Health Initiative is a voluntary service. The caregiver must be engaged in order to access services.The Children’s Mental Health Initiative’s objective is to allow families access to needed services so that children do not enter the child welfare or probation system for the sole purpose of accessing services


  • To make sure families do not get bounced from agency to agency trying to access services
  • To ensure that children are receiving services in the appropriate system
  • To build community collaborations through examining current practices and cross-educating community stakeholders on available resources
  • To construct a multi agency approach for all children within communities in order for best outcomes and service conditions


Eligibility:

  • DSM-IV-TR Diagnosis- Youth meets criteria for two (2) or more diagnoses
  • CANS 4, 5, or 6 and DMHA/DCS Project Algorithm must be a 1
  • Child or adolescent age six through the age of 17
  • Youth who are experiencing significant emotional and/or functional impairments that impact their level of functioning at home or in the community (e.g., Seriously Emotionally Disturbed classification)
  • Not Medicaid Eligible/Lack funding for service array
  • Other children who have been approved by DCS to receive services under the Children’s Mental Health Initiative because they are a danger to themselves or others

Child Mental Health Wraparound Program Information

Indiana Medicaid offers coverage for the Child Mental Health Wraparound home and community-based services. These services are provided to youth, ages 6-17, who have a diagnosis of a serious emotional disturbance. A person-centered treatment plan is built upon the member and family's strengths to identify the following:


  • The unique needs of the CMHW member
  • Services and strategies that assist the member and family in achieving more positive outcomes in their lives


Individuals must meet HCBS eligibility and Medicaid eligibility guidelines in order to be eligible for CMHW services. 


To be eligible individuals must:

  • Be 6 through 17 years of age
  • Be eligible for Indiana Medicaid
  • Have two or more Diagnostic and Statistical Manual of Mental Disorders Text Revision diagnoses that are not considered excluded
  • Demonstrate a level of need by meeting the following criteria:
    • Receive a score of 4 or higher on the Division of Mental Health and Addiction approved assessment tool
    • Have one or more of the following behavioral or emotional needs:
    • Adjustment to trauma
    • Psychosis
    • Debilitating anxiety
    • Conduct problems
    • Sexual aggression
    • Fire-setting
  • Demonstrate a significant need in at least one of the family or caregiver areas that results in a negative impact on the applicant's mental illness:
  • Mental health
  • Supervision issues
  • Family stress
  • Substance abuse
